WHAT TO EXPECT ON YOUR FIRST VISIT:
Plan for approximately 45 minutes in total, for your initial appointment.
- All new patients are required to complete and submit our online Intake Forms as soon as possible and within 4 hours of making the appointment in order to have the appointment reserved.
- If you are unable to keep your appointment, we require at least 48 hours of advanced notice, so that we can offer this appointment time to another patient. We charge a $20 fee for missed appointments.
- Bring with you a few pairs of shoes, slippers or boots that you wear most often and any old orthotics you would like Dr. Horowitz to see.
- Bring any DVDs, X-rays, Medical Reports or Laboratory results that you have in your possession.
- At the conclusion of your examination, Dr. Horowitz will share her examination findings and diagnosis with you. At that time, she will advise you of treatment recommendations and options for your condition.
WHEN TREATMENTS ARE REQUIRED:
- Hands-on care for your condition typically lasts approximately 10-12 minutes (for a regular treatment) if one area of the body is being treated and 18-20 minutes for more than one area. Longer appointments are also available.
- Dr. Horowitz teaches patients home and self-care management to expedite healing. Patients who follow healing instructions tend to improve more quickly and require a fewer number of treatments overall.
PAYMENT PROCEDURES:
- It is our clinic policy that payments must be made in full at the time services are provided. If you require custom orthotics, payment in full is required prior to placing any manufacturing orders.
INSURANCE INFORMATION:
- Our treatments are covered by Extended Health Insurance and billed under Chiropractic/ paramedical services. Dr. Horowitz is registered as a Doctor of Chiropractic. She is a registered provider of custom foot orthotics with most Insurance companies.
- Custom orthotics are covered by most Extended Health Insurance plans.
- We are happy to Direct-bill for examinations and treatments when possible and provide you with Insurance forms needed for reimbursement for custom orthotics.
- Provincial Insurance (OHIP) does NOT cover the cost of custom orthotics or treatment services.
- We offer the service of submitting the initial examination fee to your insurance provider on your behalf, to expedite reimbursement.
- For custom foot orthotics, insurance companies expect their clients to submit the required forms directly. We will assist you and provide you with the paperwork and reports that insurance companies require.
- Once we receive your custom orthotics from the lab, we will call you to book a dispensing appointment and discuss your follow-up for your treatment plan. The dispensing appointment is usually within one to two weeks of your initial assessment and only takes 5-10 minutes. There is no fee for this appointment. If you are coming in for treatment, please also book a separate dispensing visit, so we have more time.
- It is the patient’s responsibility to contact his/her extended health insurance company to be certain about the details of coverage. In order to be reimbursed for custom orthotics, some insurance companies specify that they require your medical doctor to write the prescription, as there are some companies that do not accept one from a Chiropractor.
WE STAND BEHIND OUR PRODUCTS:
- We offer a 3-Month Modification Warranty: Occasionally modifications are required to ensure the right fit. If any modifications/adjustments to the orthotics that you ordered are required within the first 3-months, there is no extra charge to you.
